Locals report to Regionals. Use the online form.

For the December or Domesday report ALL Chirurgeons should also send one set to Regional and one set to Kingdom of photocopies of the following documents:

• Current C.P.R. Certification
• Current First Aid Certification
• Current Membership
• Proof of age of majority.

A December/Domesday is REQUIRED for all Chirurgeons to remain warranted.

Note: Photocopies of new C.P.R. and/or First Aid certifications should ALWAYS be sent to Kingdom as soon as they are acquired.
