[Mid] Lost heraldic submissions

Someone posted: <Well my heraldry is not officially registered since it was one

I'm just curious how many times it's happened that submissions have been lost
on a wholesale scale. The poster implied that her submission was one of many
lost at the same time. Our local herald has taken A LOT of heat for losing the
submissions of many newbies in our group. Whether or not it was her fault, I'm
curious to know how often this occurs.
