number, email, awards and other information that you may want to 
publish on the Templemede website.  If any of your information has 
changed since the date after your name, please contact me and we will 
get another waiver filled out.

You may indicate on the form if there is any specific information 
that you DO NOT wish to be published on this webpage (such as your 
real name), and your request will be honored.  It is my policy as 
webministress to keep Mundane names off the internet unless that 
person is in a publicly accountable position, such as an Officer or 
an Autocrat.

Anyone else who considers themselves part of Templemede and wishes 
to be included on the "Officers and Personages" page needs to also 
let me know and get a waiver signed and on file.  In addition, if there is a star after your name, I need to have your waiver replaced in my files.  Please email me at alliquay@midrealm.org and we can set up a way to get your paperwork signed.
